1. TOO FREQUENT MEETINGS WITH NO AGENDA

2. POOR CHAIRMANSHIP – NO CONTROL OF MEMBERS

3. TOO MANY INTERRUPTIONS OR DISTRACTIONS FROM MEMBERS AND CHAIRMAN; NOT KEEPING TO AGENDA

4. RELEVANT PERSONS NOT PRESENT AT MEETINGS – TO PREDETERMINE ANT KEY PERSONNEL PRESENT

5. LACK OF PROPER INSTRUCTIONS ON CONDUCT OF MEETING AND TIME ALLOCATED FOR EACH MATTER TO BE DISCUSSED

6. SLOW DECISION MAKING – LACK OF FULL RELEVANT DATA; TOO MUCH DATA COLLECTION, INADEQUATE SUMMARY OF IMPORTANT POINTS
